Community Health Advocates
Helping People Get, Keep and Use Health Coverage
Background
In March 2010, the federal government passed a health care act reform bill called the Affordable Care Act. One important part of the Affordable Care Act is Section 1002, which provides federal funds to support direct assistance to consumers with enrolling in and using insurance coverage.

What services do the Community Health Advocates provide?
Community Health Advocates provide one-on-one counseling and community training about health insurance and access to care. Consumers get help in their communities whether they are privately insured, publicly insured, or uninsured.

What does this mean for you?
The North Country Prenatal/Perinatal Council is able to offer a Community Health
Advocate (CHA) to residents of Jefferson, Lewis and St. Lawrence counties to assist
people with their insurance needs.
A CHA can present to your group!
Presentation topics include:
- · Accessing health insurance
- · Medicare, Medicaid, Child Health Plus and Family Health Plus
- · Purchasing Commercial Insurance
- · Consumer’s options and rights under the Affordable Care Act
- · Employer’s options and rights under the Affordable Care Act

A CHA can assist you!
Examples of assistance include:
- · Finding and enrolling in affordable health insurance
- · Finding low-cost healthcare alternatives for the uninsured
- · Locating health care providers
Assistance dealing with medical bills

The Community Health Advocate can also refer people to Facilitated Enrollers
Facilitated Enrollers are specially trained to help people enroll in Medicaid, Child Health Plus and Family Health Plus. There are Facilitated Enrollers in Jefferson, Lewis and St. Lawrence Counties.
