What is Facilitated Enrollment?
Facilitated Enrollment is a program to help people in the North Country get health insurance for themselves and/or their children. It offers an easy way for people to learn about the available health insurance programs and help them find out if they or their children are eligible. Enrollers can be found in a local community setting and will work one on one to help individuals complete an application, gather necessary documentation and review eligibility for Family Health Plus, Child Health Plus, and Medicaid.

Children under the age of 19 may be eligible for either:
Child Health Plus “A” (formally Children’s Medicaid)
Or Child Health Plus “B”

Adults between the ages of 19-64 may be eligible for either:
Medicaid
Or Family Health Plus

What is Family Health Plus?
Family Health Plus (FHP) is New York State’s health insurance program for adults who do not have health insurance and have incomes too high to qualify for Medicaid. FHP can sometimes help with the cost of premiums for those with employer sponsored insurance and who are FHP income eligible! FHP is available to single adults, couples without children, and parents with low income.

What is Child Health Plus “A”?
Child Health Plus A, formerly known as Medicaid, is a free health insurance program for children under age 19 that offers complete coverage for all of their health care needs. Eligibility is based on income and family size.

What is Child Health Plus “B”?
After July 20th, applications will be taken for September 1st coverage for expanded CHP coverage. Families with incomes up to 400% of the Federal Poverty Level will now be eligible for subsidized premiums. For example, a family of 3 with one child could make up to $5,867 per month and only pay $40.00 per month for insurance!

What Services will I receive with Family Health Plus, Child Health Plus, or Medicaid?
Inpatient hospitalization, emergency care, primary and preventative care, vision, dental care, well child care, prescriptions and more.

How Do I Know If I am Eligible?
Our Facilitators are trained to screen for eligibility. Based upon the income information you provide, an initial review for eligibility and assistance to complete the necessary application will occur.

Adult Medicaid and Family Health Plus also require that a resource screening be completed. Our facilitators are trained to help you with this process. There is no resource test for children!

In Lewis County:

The Facilitated Enrollment Program, a NYS DOH initiative, was implemented in April 2000 by NCPPC. This year the three northern counties were provided enrollment services through 2 subcontracts to community agencies in the tri-county area (North Country Children's Clinic and Planned Parenthood of Northern New York) and through NCPPC. Families receive assistance with the Medicaid, Family Health Plus, and Child Health Plus application process. Since the beginning of the new contract in January 2003, 19,582 children and 6,669 adults have been enrolled in the tri-county area.
