Community Health Worker Supervisor Position – Full Time

 

 

The North Country Prenatal/Perinatal Council is seeking a full time Community Health Worker Supervisor. This position is responsible for providing supervision and support to a team of Community Health Workers (CHWs) working within Jefferson, Lewis and St. Lawrence counties to outreach and support hard to reach community members and engage them in appropriate health services.

 

 

Responsibilities:

  • Provide 1:1 reflective supervision for the CHWs.
  • Support CHW team to conduct basic health assessments, identify needs, and provide basic health information.
  • Make appropriate referrals to preventive and primary health care services through home visits and street outreach.
  • Plan and coordinate outreach, supervise individual and family assessments, assess training needs and provide training for CHWs as needed.
  • Facilitate case conferencing with CHW team to discuss client cases and provide support.
  • Develop, in conjunction with the MICHC Program Director, an overall work plan, including outreach strategies.
  • Collect data and create quarterly and yearly reports or as required by contract.

 

 

Qualifications:

  • Licensed Social Worker (LMSW or LCSW) or Nurse (BSN) required with valid NYS license.
  • 2 years’ experience providing clinical, 1:1 reflective supervision preferred with knowledge of home visiting preferred.
  • Must be skilled at supervising, counseling, teaching, and motivating staff.
  • Must have outstanding oral, written and interpersonal communication skills.
  • Computer literate; proficient with Microsoft Office.
  • Familiarity with the geographic area and the population to be served.
  • Be available on an “on-call” basis to provide consultation to CHWs working with families in crisis.
  • Bilingual applicants will be given preference.

Health Insurance Navigator (St. Lawrence County)

 

 

The North Country Prenatal/Perinatal Council is seeking applicants for two Health Insurance Navigator positions in St. Lawrence county. The successful candidates will assist consumers to apply for and enroll in health insurance through the New York State of Health Marketplace, which includes Qualified Health Plans, Child Health Plus, the Essential Plan and Medicaid.

 

 

Qualifications: two years of experience in NYS benefit programs or health insurance experience, or other related experience; associates degree in related field (appropriate work experience will be considered in lieu of education); ability and strong desire to work with diverse socio-economic populations; ability to work independently; must have strong computer literacy abilities, excellent interpersonal and organizational skills with exceptional attention to detail. Must have valid driver’s license and reliable transportation.

 Sexual Health Educator

The North Country Prenatal/Perinatal Council is seeking qualified candidates for a Sexual Health Educator position based primarily in Jefferson County, NY. This full-time position is responsible for the implementation of a teen pregnancy prevention program in accordance with stated goals and objectives. The Educator provides school-based and some community comprehensive sexual education components; maintains open and consistent communication with school district staff; facilitates community relations including public education, outreach and marketing initiatives to raise community awareness of adolescent issues including pregnancy; identifies at-risk populations and issues related to service delivery; and works with special populations including military dependents, LGBTQ youth, and high need populations. Candidates must have knowledge of sexual health, adolescent development and issues, community systems and services, and youth development/assets model concepts. Candidates must possess the skill set required to work positively with youth and young adults both as individuals and in groups; provide adolescent sexual health programs at Middle and High School levels; and, demonstrate strong written, personal and electronic communication and public speaking skills. To be considered, candidates must possess BA/BS in a related field (ex., community health, education, human services) and experience in health education, particularly sexual health education, and/or youth work. Personal vehicle, valid driver’s license, and ability to pass a background check required.

 

 


Community Health Worker

This position is located in and will serve St. Lawrence County

 

 

The North Country Prenatal Perinatal Council, Inc. is seeking Community Health Workers to improve population health in targeted communities across Jefferson, Lewis, and St. Lawrence counties.

 

CHWs will coordinate direct services and provide healthy lifestyles support to individuals with chronic health issues or in need of additional help to avoid re-hospitalization or emergency room use.  Supports may include assistance with developing a wellness plan, education, assessment of barriers to healthy living, access to health insurance and insurance literacy.  Principal responsibilities include participant recruitment, home visits promoting wellness, referral to community resources and programs, coaching, and other individualized supports.

 

Qualifications include high school diploma or equivalent, regard for patient privacy, reliable transportation, good communication skills, experience with data management, and a flexible schedule.  Bilingual or American Sign Language is preferred.
